Job Description Position Title: Intensive Care Coordinator (ICC)
/ Clinician Youth Homes is looking for an Intensive Care
Coordinator (ICC) / Clinician to join our Family Pathways Program.
This position is mostly case management with the opportunity to
provide individual or family therapy or other mental health
services to meet youth and families' emotional and mental health
needs that support successful family relationships. This position
is an excellent opportunity for someone that has their BBS
registration and needs to gain their hours toward licensing. Note:
This position is a combination of fieldwork with clients, office
for meetings and training, and remote for case management

Responsibilities Provide case coordination, therapy, and advocacy
for 10-15 children, youth, and families to promote healing and
re-connection Under the supervision of a licensed professional,
provide mental health assessments and comprehensive treatment plans
for your assigned caseload Facilitate Child and Family Team (CFT)
meetings/interdisciplinary community case coordination meeting
Provide daily monitoring of client/family needs and offer case
management as needed Develop and maintain a consistent appointment
schedule with clients to enhance therapeutic alliance and treatment
teams to encourage collaboration Provide modeling and coaching of
prosocial interpersonal communication, coping strategies, and
independent living skills Recognize and effectively navigate team
dynamics to ensure optimal client outcomes Maintain agency
productivity standard, currently 22 billable hours per 40-hour
workweek Utilize Electronic Health Records efficiently and
effectively to complete all required documentation for Medi-Cal
compliance promptly Adhere to all agency policies and procedures,
including NASW Code of Ethics, HIPAA, Core Competencies, and Code
of Conduct, Employee Handbook, and state and federal regulations
Enjoy working in a diverse, multi-cultural environment and be
sensitive to the service population---s cultural and socioeconomic
characteristics Know the foster care system in California and be
familiar with trauma-informed care Maintain a professional manner
at all times while representing the agency Minimum Qualifications
and Skills Licensed or license eligible MFT or LCSW with BBS
registered number in good standing. Associate MFT or Associate MSW
is also acceptable Experience working with at-risk children and
adolescents in a community-based environment Responsible for
providing own well maintained and privately insured car for daily
travel to clients and office. Experience with family systems and
field-based family behavioral engagement and treatment Demonstrates
excellent interpersonal skills and ability to communicate clearly
and professionally with clients, caregivers, family members, and
other professionals Possess exceptional clinical judgment and
problem-solving skills to be able to manage clinical crises and
complex case scenarios Excellent documentation skills Competent in
Microsoft Office Suite products Coachable and committed to
improving personal knowledge, skills, and abilities Have the
ability to navigate a fast-paced, constantly changing daily
schedule The physical demands described here represent those that

frequently is required to stand, walk, sit, move quickly/run, use
hands, handle, or feel, reach with hands and arms, stoop, kneel,

required to climb or balance. The employee must frequently lift and
move up to 20 pounds. This job's specific vision abilities include
