-Caregivers, -Certified Nursing Assistants or -Direct Support
Professionals -to work with our team in residential group homes.
This is a non-exempt level position reporting to the Program
Administrator. -POSITION SUMMARY: The Certified Nurse Assistant
(CNA) assists residents with personal care activities,
nutritionalneeds, and restorative function. CNAs perform assigned
medical tasks, provide individual transportation, and assistwith
leisure activities. Emphasis is placed on active treatment goals
and achievement of the highest level of function for -the resident.
CNAs adhere to clinical standards of care and organizational
policies and procedures. -DUTIES AND RESPONSIBILITIES:Personal Care
Activities--- Performs/assists with personal care activities
including bathing, dressing, shaving, toileting, and incontinence
care inaccordance with program protocols--- Performs/assists with
oral care including brushing and flossing of teeth, swabbing gums,
denture care, andadministration of mouthwash--- Performs/assists
residents with nail care of finger and toenails for non-diabetic
residents--- Assists residents with snack and meal set up and
feeding including but not limited to utilizing prescribed
diettextures and consistencies, feeding techniques, and adaptive
feeding equipment -Restorative Care Activities--- Assists residents
with ambulation including use of gait belts, walkers, rollators,
and other devices in accordance withthe resident's plan of care---
Applies prescribed braces--- Assists residents with transfers to
and from bed, chair, or wheelchair utilizing two-person transfer
techniques andmechanical lifts as needed--- Maintains proper
positioning of residents in bed, therapeutic chair, or wheelchair
to prevent aspiration--- Maintains every two (2) hour (Q2) turning
and repositioning for all assigned residents including utilizing
positioningpillows and tilt in space chairs to relieve pressure and
documenting position changes on turning and repositioningworksheets
-Monitoring of Physical Status--- Takes vital signs for residents
as specified by the care plan and records the results in the
resident's chart--- Empties Foley catheters and documents output---
Applies prescribed oxygen therapy devices such as nasal cannula or
oxygen mask and monitors placement--- Monitors resident bowel
function and maintains documentation of bowel movements,
documenting changes inbowel function and alerting the nurse---
Observes resident for changes in baseline level of mental and
physical function and immediately reports changes tothe licensed
nurseDocumentation and Reporting--- Completes progress notes,
supports/goals data collection sheets, Transfer Sheet (for
recording transfer of coveragefor 1:1 assignments), Treatment
Administration Record (TAR), and Behavioral Support Plan (BSP)
documentation asassigned -NCN013 - Certified Nursing Assistant
Created October, 2021--- Completes Incident Reports per Elwyn
policyOther Duties--- Maintains a clean-living area for residents,
including the dining areas and kitchen--- Actively carries out
building safety routines and takes the initiative to correct safety
hazards or reports hazards toproper supervisory personnel---
Maintains resident's room in a neat and orderly manner, ensuring
that all equipment is functional and in workingorder;
individualizes each resident's personal environment--- Ensures that
resident's personal equipment/specialized devices are clean, in
good working order, and used correctly.Reports malfunctions,
missing, or broken items to supervisor--- Transports residents to
and from day programs, recreational activities, unit based events
and medicalappointments/procedures--- Trains and assists residents
in constructive use of leisure time. Encourages participation in
planned recreationalactivities--- Assists with shopping for
residents including but not limited to purchasing clothing or
personal items for residents'rooms--- Participates as a member of
the Interdisciplinary Treatment Team by contributing to the
development of behaviormanagement and program plans developed for
each resident and monitoring individual progress--- Provides
assigned coverage for co-worker's residents if co-worker is on
break or needs to leave the building--- Actively participates in
unit-based meetings and in-services, clinical rounds, quality
improvement activities,educational training--- Remains on duty
until relieved by co-worker--- Attends training and in-service
opportunities as scheduled--- Performs cardiopulmonary
resuscitation (CPR), and crisis intervention using agency-trained
protocols--- Performs other duties as assigned -IMMEDIATE
SUPERVISOR: Program Administrator. May take direction from lead
staff on their scheduled shift.DIRECT REPORTS: None.CONTACTS: Elwyn
personnel, residents, funding and regulatory agency personnel,
parents/conservators and family,visitors, outside vendors,
contractors, etc. -EDUCATION/EXPERIENCE/SKILLS REQUIREMENTS:---
High School diploma or equivalent--- Current CNA certificate---
Maintain CPR and First Aid certification--- One (1) year of
experience working with individuals with developmental
disabilities--- Demonstrated ability to work effectively as part of
a team--- Ability to work in a fast-paced environment and manage
and prioritize multiple, often competing, priorities---
Demonstrated strong attention to detail--- Demonstrated ability to
anticipate future needed changes or identify problem areas and take
effective actions--- Must be able to perform CPR, lifting and
transferring, and crisis intervention techniques using
agency-trainedprotocols--- Must possess excellent customer
interaction, collaboration, and written and verbal communication
skills--- Demonstrated basic experience with internet-based
programs--- Knowledge of Electronic Health Record (EHR)
preferred--- Must have current, valid driver's license in state of
residence, three (3) years driving experience in the United States,
-and acceptable driving record; otherwise-qualified candidate may
be considered for non-driving position with less -than three years
of driving experience in the United States to be converted to a
