- Job Summary: The WelbeHealth Occupational Therapist is
responsible for coordinating and providing restorative and
rehabilitative Occupational Therapy services to participants at the
Center and at home. Occupational Therapy works closely with PACE
Physician, rehabilitation staff and other interdisciplinary team
(IDT) members to maximize participant independence and safety as
well as enhance performance of ADLs. The Occupational Therapist
collaborates with the Physical Therapist to efficiently operate
WelbeHealth rehabilitation department and is a member of the
WelbeHealth management team and facilitates collaborative
relationships across WelbeHealth and with external stakeholders to
promote the WelbeHealth values, team culture and mission.
- Conduct Occupational Therapy assessments to determine the
Occupational Therapy care needs, preferences and goals of the
participants and actively participate in IDT meetings to develop
participant care plans.
- Deliver and document Occupational Therapy interventions in
accordance with physician orders and/or as agreed upon in the
participants' care plans including but not limited to the use of
devices, techniques and activities to promote muscle re-education,
perceptual-motor and cognitive function, balance, endurance and
other factors affecting the level of independence in performing
ADLs.
- Supervises the Therapy Assistants including providing training,
instruction and guidance needed to implement Functional Maintenance
Programs (FMP).
- Provide training and instructions to staff, participants and
caregivers/families in areas of Occupational Therapy in which they
may work with participants.
- Order, track and manage maintenance for equipment.
- Remain alert to pertinent input from other team members,
participants, and caregivers and update the IDT promptly of any
changes in participants' condition or functional status.
- Work with the other members of the IDT to support smooth care
transitions between settings (e.g., hospitals, skilled nursing
facilities, home, etc.).
- In coordination with the Marketing Team, support enrollment of
prospective participants into the program.
- Participate in end-of-life care coordination and support.
- Manage direct reports including training, supervising and
mentoring.
- Be legally authorized to practice in position location,
required.
- Only act within the scope of his or her authority to
practice.
- Complete all required documentation in a timely and accurate
manner.
- Protect privacy and maintain confidentiality of all company
procedures and information about team members, participants, and
families.
- Follow WelbeHealth policies and procedures and participate in
any required Quality Improvement activities, staff training and
meetings.
- Communicate regularly with Supervisor and team regarding
workload and priorities.
- Timely completion of all mandated trainings and education.
- Timely completion of all mandated occupational health
screenings as needed.
- Exercises flexibility in performing assignments as business
needs evolve.
- Other duties as assigned.
- Job Requirements
- Minimum of two years Occupational Therapy experience in a
clinical setting with a frail or elderly population.
- Highly motivated, inspirational leader who is able to
innovatively engage, support and remove barriers for the team to
ensure the team consistently meet established goals.
- Occupational Therapy knowledge and skills necessary to treat
frail, elderly participants with complex needs.
- Two years' supervisory experience with demonstrated ability to
mentor and develop team members.
- Ability to process accurate information in a timely
manner.
- Excellent leadership, organizational and communication skills
in settings with seniors, their families and interdisciplinary team
members.
- Experience leading in a data-driven organization, leveraging
reports and data to prioritize and manage people and projects.
- Excellent clinical, organizational and communication skills in
settings with seniors, their families and interdisciplinary team
members.
- Ability to work independently with minimal supervision.
- Demonstrated ability to prioritize in a fast-paced
environment.
- Experience and competency working with people from diverse
backgrounds and cultures.
