Job Description:The Home Health registered nurse, working with minimal supervision, coordinates all aspects of the patient's home care, interacting with other disciplines as needed, and provides professional care to the patient as indicated by agency policies, State and Federal laws and regulations. The person in this role determines patient's eligibility for home care services and develops the plan of care to be followed. He/She regularly evaluates and communicates the patient's significant changes in condition and addresses them appropriately. The Home Health Nurse completes all required documentation on a timely basis according to Agency guidelines.Education:

• Graduate of an Accredited School of Nursing RequiredExperience:
  • 1 year professional nursing Required
  • 1 year Healthcare - Acute Care and/or Healthcare - Home Care PreferredFor Maternal/Neonatal/Pediatric Nurses:
    • 1 year professional nursing Required
    • 1 year post partum and/or NICU/PICU Required
    • Experience in Healthcare/Medical - Pediatrics PreferredFor Adult Care Nurses:
      • 1 year professional nursing Required
      • Experience in Healthcare - Home Care or Healthcare - Acute Care for Adult Nursing PreferredCertifications/Licensures:
        • Registered Nursing License (RN) - Current in the state working in -Required
        • BLS Basic Life Support - American Heart Association -Required
        • Driver's License - Department of Motor Vehicles -Required
        • Car Insurance - No Issuing Agency -Required
        • International Board Certified Lactation Consultant - International Board of Lactation Consultant Examiners Preferred---Skills:
          • Ability to work non-judgmentally and communicate professionally with persons of varied cultural, religious and socioeconomic backgrounds and lifestyles.
          • Demonstrated ability to be self-directed, and motivated. Demonstrated ability to work independently and as a part of a multidisciplinary team.
          • Excellent written and verbal communication skills. Excellent and effective oral and written communication skills to successfully communicate well with all levels of management, staff, faculty, and clients both inside and outside the organization.
          • A high degree of organizational skills, ability to set priorities, manage multiple demands and the ability to complete tasks under strict time lines is required.Work Shift:08.0 - 08:00 - 16:30 No Waive (United States of America)Pay Range:$64.40 - $90.16HourlyOffer amounts are based on demonstrated/relevant experience and/or licensure.Note: Positions at JMH which are exempt (not eligible for overtime) under the level of Manager are listed as hourly for compensation purposes on this posting.
