Job Profile Summary
The Behavioral Health Navigator (BHN) is a patient care coordinator who helps connect patients with appropriate treatment and services to address problems related to mental health conditions, their use of alcohol and other addictive substances, or a combination of both. Behavioral Health Navigators conduct initial brief assessments, introduce patients to behavioral health treatment programs and services, expedite appointments with appropriate providers, serve as the primary coach for their clients, and maintain ongoing contact with their panel. They also assist with access to other services such as financial counseling, primary care, social services, and residential treatment facilities. The Behavioral Health Navigator proactively builds and cultivates a network of service providers within the John Muir Health system, as well as with various private and public behavioral health treatment resources throughout the community. The Behavioral Health Navigator acts as a subject matter expert in behavioral health treatment options and resources for mental health conditions, substance use disorders (SUD), and co-occurring mental health and substance use disorders. Additionally, the Behavioral Health Navigator contributes to the development and strategic planning for the BHN program by helping to implement JMH and grant-funded initiatives, active membership on relevant committees, and participation in BHN outreach and training efforts and presentations.

Education:

• High School Diploma and Certificate - Addiction Studies / Addiction Counseling - Required or
• High School Diploma and Two (2) years Work Experience - Full Time Addiction Medicine - Required Experience:
  • 2 years Substance Use Disorder Treatment - Required
    Certifications/Licensures:
    • BLS Basic Life Support - American Heart Association - Required
    • Crisis Prevention Institute (CPI) Training - John Muir Health - Required within 60 days of assuming position with renewal every 24 months ( Existing employees have 60 days from 6/13/2022 to complete the CPI Training ) Skills:
      • Nonjudgmental, energetic, positive approach to assisting patients with Substance Use Disorder (SUD)
      • Understands SUD as a medical condition and the role of Medication Assisted Treatment (MAT)
      • Does not rigidly adhere to an abstinence-based social model approach, but adapts approach to each patient's individual needs and circumstances, including harm reduction when appropriate.
      • Ability to apply critical thinking skills, ability to multi-task and manage time and work assignments independently.
      • Possess the ability to work independently as well as part of a team.
      • Demonstrated ability to lead and facilitate diverse groups of people with understanding of group psychodynamics, being able to maintain control over the overall needs of the group; aware of each individual's needs simultaneously.
      • Ability to work respectfully and creatively with clients of diverse functional abilities, social, economic, and cultural backgrounds to support both client autonomy and client safety.
      • Sensitivity to diverse religious and cultural practices and values.
      • Ability to work cooperatively and collaboratively with staff at all levels across the organization.
      • Excellent written and verbal communication skills across organizational levels.
      • Must be teamwork oriented and skilled at building constructive and effective relationships.

        Pay Range:
        
        $30.38 - $41.02 Hourly
        Offer amounts are based on demonstrated/relevant experience and/or licensure.
        
        Note: Positions at JMH which are exempt (not eligible for overtime) under the level of Manager are listed as hourly for compensation purposes on this posting. The work shift will contain the word 'exempt' on it.
